HCSOM Pilots Kick Off with Anniversary Training

The HCSOM pilot phase of the S4H project was launched with a high-level accredited training in Bicske, marking the second anniversary of the Maltese Charity Service’s telemedicine programme. Since 2023, over 12,000 patients have been supported in underserved areas by doctors using mobile clinics and telemedicine. On 6 March, more than 60 doctors participated in the training to strengthen healthcare delivery in Catching Up Settlements.

HCSOM Pilots Expand with Telemedicine Measurement

Starting in May, the HCSOM pilots of the S4H project have entered a new phase with the inclusion of telemedicine appointment measurements. The pilot actions are built on three pillars: education, testing of innovative care models, and patient follow-up. As part of this expansion, we have begun collecting data and testing tools for non-communicable disease (NCD) screening and care in participating municipalities.

HESED brings health promotion back to the spotlight

In May 2025, HESED relaunched its community-based health promotion and screening activities, with a special focus on Roma communities. Newly trained outreach workers now provide waist and blood pressure measurements, risk assessments, and referrals to medical services, including support for uninsured individuals.

As part of the World Hypertension Day campaign, more than 45 cardiology appointments were arranged with free examinations and transportation support.

WP3 Nears Completion – Training Materials Finalised

Work Package 3 (WP3) of the ShowUp4Health project is approaching its conclusion. Since autumn 2024, the consortium has been working intensively to develop four training programmes and one strategic toolkit designed to support pilot training implementation across Europe.

The following materials have been finalised:

  • Training programmes and materials for Healthcare Professionals

  • Training programmes and materials for Healthcare Mediators

  • Training programmes and materials for Social Workers

  • Training programmes and materials for Ukrainian professionals and NGO outreach workers on mental health screening

  • Strategy and Toolkit for Engaging Local Stakeholders in support of vulnerable communities

These resources incorporate contributions from all consortium partners, drawing on existing national programmes and best practices.

The finalised materials cover a wide range of relevant content, including communication techniques, first aid, internationally validated methods for NCD screening, effective practices with disadvantaged groups, and tailored support for social workers engaged with vulnerable populations.

These training materials will serve as the foundation for the pilot training sessions to be delivered later this year.

Care in the Open Air: Health Awareness Event in Uzhhorod

On 29 April 2025, Teatralna Square in Uzhhorod hosted a vibrant open-air health awareness event as part of the SHOW4Health Project. Organised by the InterFamily Clinic in partnership with Uzhhorod National University’s Department of Family Medicine and Outpatient Care, the event aimed to promote preventive healthcare and inspire greater attention to personal well-being.

The day featured the introduction of CheckMe, a digital tool offering personalised health screening recommendations based on age, gender, lifestyle, and medical history — all aligned with global clinical guidelines.

Participants were offered free consultations, health screenings (including blood pressure, blood sugar, and BMI checks), and information on risk factors, healthy living, and the importance of preventive care. Special focus was placed on supporting internally displaced persons (IDPs), alongside young people, middle-aged adults, and older residents.

The event was not only a public health milestone but also a community celebration of awareness, science, and shared responsibility. Our heartfelt thanks go to all participants — together, we are building a culture of health-conscious living.

Improving Healthcare for Displaced People in Ukraine: The “InterFamily” Pilot Project

The Family Medicine Clinic InterFamily in Uzhhorod, Ukraine, is leading a ground-breaking pilot project under the SHOW4Health initiative to improve healthcare for internally displaced persons (IDPs). Focused on early screening for non-communicable diseases (NCDs), including cardiovascular conditions, diabetes, and mental health challenges, the project combines medical training, digital innovation, and community partnerships.

Healthcare professionals receive short, targeted training sessions to enhance their skills in NCD screening, chronic disease management, and rehabilitation. Mental health is also a key priority: tailored screening tools and a referral system ensure IDPs receive the support they need.

A digital tool – CheckMe, a web-based screening advisor – supports healthcare providers in identifying hidden diseases and offering personalised recommendations.

Thanks to the SHOW4Health grant, the project funds a multidisciplinary team and a mobile unit caring for elderly patients. Collaboration with local NGOs and continuous evaluation ensures accessible and sustainable care for displaced populations.

InterFamily presented its results at the 100th EGPRN Conference in Sweden and recently held an open-air outreach event in Uzhhorod to raise awareness and reach more people.

ShowUp4Health – WP4 Pilots Are Underway!

The ShowUp4Health project has reached an exciting milestone in Work Package 4 (WP4) – Co-Planning. After months of intensive preparation and coordination, pilot activities have officially begun across the participating countries. Local partners are now actively implementing their tailored actions, aiming to strengthen collaboration between health and social sectors and to test effective models for local-level NCD (non-communicable diseases) prevention and screening.

The groundwork was laid through a structured planning process, including the use of a shared WP4 Piloting – Planning Form to identify pilot objectives, key stakeholders, required resources, and evaluation methods. The planning phase was supported by regular online and in-person meetings to ensure local relevance and sustainability.

In the coming period, we can expect:

  • Involvement of health and social care professionals

  • Training of new health mediators and field workers

  • Launch of screening activities and awareness campaigns

  • Use of digital tools to support monitoring

  • Sharing of early results and lessons learned

  • Strengthening stakeholder collaboration

  • Scaling up and sustainability planning

Integrated Social-Health Development Methodology has been developed

In recent months, one of the most decisive professional deliverables of the ShowUp4Health project has been developed: the Integrated Social-Health Development Methodology (ISHDM), which defines the framework for pilot activities planned for a later phase of the project. The most important feature of ISHDM is its adaptability and flexibility to local conditions. The methodology is designed to accommodate and respond to the diverse and specific needs of different vulnerable populations.
The methodological material aims to take a holistic approach to prevention, screening, the continuum of care, and professional collaboration. Roma communities and displaced people face complex and overlapping barriers to accessing healthcare. Their experiences are characterized by socio-economic deprivation, cultural exclusion, systemic discrimination, and inadequate health infrastructure.
Each target area – Hungary, Romania, Bulgaria, Slovenia, and Ukraine – presents unique challenges that require context-specific strategies to address inequalities. ISHDM is designed to take these specificities into account and facilitate the development of equitable and sustainable health solutions tailored to local needs. A deep understanding of the needs and challenges faced by users and target groups served as the foundation for the first baseline document.
Our main objective is to improve the quality of life of vulnerable communities, especially Roma, in specific areas related to non-communicable diseases. To achieve this, we are developing supportive intervention methods to be tested in different settings.
Key elements of the methodological material include:
    • Concrete actions in prevention, screening, and follow-up
    • Process descriptions, training, education, and workshops
    • Telemedicine opportunities
    • Engagement of lay facilitators
    • Targeted communication campaigns to enhance health literacy and increase participation in screenings
The methodology focuses on leveraging existing resources to enhance collaborative effectiveness. These existing resources include stakeholders, professionals, and lay facilitators working with the target populations. Emphasis is placed on:
    • Activation – engaging and mobilizing stakeholders
    • Strengthening and maintaining connections
    • Expanding and systematizing knowledge
A key feature of the material is its focus on IDPs in Ukraine, where health screenings are complemented by mental health assessments, including screening and follow-up for depression and anxiety.
